I understand that there are risks of physical injury associated with, arising out of and inherent to the activity of dance. In recognition of this acknowledged risk of injury, I knowingly and voluntarily waive all right and/ or causes of action of any kind, including any and all claims of negligence arising as a result of such activity from which liability could accrue to MP Dance Company and it’s affiliates. I hereby agree to release MP Dance Company and hold MP Dance Company harmless of all liability, and hereby acknowledge that I knowingly and voluntarily assume full responsibility for all risks of physical injury arising out of active participation in dance on behalf of the participant. The participant has my permission to participate in MPDC Events. I warrant the above information is complete and correct. I further release MP Dance Company of all liabilities associated with my child’s attendance at MPDC. (please type signature below.)

We are so excited to have your dancer and we have such a fun year planned!

To secure your spot in class please send payment for your uniform fee and first months tuition. Payments can be sent several ways:
-via venmo to @mecinna-price (preferred, please note "Sept dance lessons" on the payment.)
-paid through our website on the "make a payment page" (you will notice a couple dollars added to each payment to account for paypal fees)
-mailed to:
MP Dance Company

Fall classes will begin Monday, September 10. Please have shoes purchased, hair pulled up for class and wear your own dance attire or a tank and shorts for the first couple weeks. Uniforms will be ordered once we can measure and fit students. Level 1 parents are welcome to stay for the first few minutes of class but we do ask that you try to leave class and wait in the lobby or run an errand so the tiny dancers have time to acclimate to their new teacher and classmates. We promise, they do better when you are not there and we will let you know if your dancer is needing you :)
